• • Facilitate, implement, and evaluate individualized Patient Plans of Care and corresponding outcomes.

• • Deliver timely and effective nursing interventions based on patient needs.

Documentation & Communication

• • Accurately document assessments, interventions, patient responses, and provider communications in real time.
  • Perform ongoing review, initiation, and implementation of provider orders each shift.

Patient Safety & Risk Assessment

• • Identify patient safety risk factors, including fall risk, skin integrity concerns, and the need for restraints.
  • Implement appropriate safety interventions promptly and consistently.
  • Follow medication safety policies and complete all required Safety Tracking Tools.

Delegation & Team Leadership

• • Delegate tasks appropriately and supervise assigned staff to ensure high?quality patient care.
  • Take responsibility for all care provided to assigned patients.
  • Report performance concerns to maintain a safe, supportive, and effective care environment.

Patient & Family Education

• • Assess educational needs of patients and families, providing clear written and verbal instructions.
  • Document all education delivered and evaluate understanding as appropriate.

• • Prioritize tasks efficiently based on urgency, patient acuity, and available resources.
